We are seeking an individual with the strong people skills, experience, and ability to join us as:
Licensed Behavioral Health Clinician
Outpatient Mental Health/Behavioral Health
Presence Behavioral Health, Aurora, Illinois

Reports to: Director

Major Responsibilities / Functions

The professional we select will assess patients admitted to Behavioral Health. This individual will gather and analyze information; recommend treatments; communicate pertinent information to other members of the interdisciplinary team; collaborate in the development of treatment plans; and coordinate or provide services, including individual therapy; group; and, discharge planning. Additional functions include:
* Participating in hospital and departmental planning committees and teams.
* Providing/coordinating a continuum of treatment services on-site or within the community.
* Performing comprehensive diagnostic assessments of patients' needs and psychological status and consulting with patient families to obtain relevant background information.
* Developing a comprehensive, individualized patient treatment plan and goals and providing individual, group and family therapy.
* Participating in multidisciplinary patient-care conferences to discuss information obtained from patient assessments, updates, and treatment plans.
* Providing documentation of all pertinent information obtained from interviews, consultation with other professionals, referrals, contacts, and patient supportive services.
* Completing documentation and reporting it in accordance with departmental standards.


Required Credentials:
* Master's degree in social work, psychology or a related field.
* Status as a licensed clinical social worker (LCSW) strongly preferred, LSW, LPC, or LMFT.
* Crisis Prevention Institute certification.
* Proficiency in Microsoft Office software (Word).

Preferred Credentials:
* Two or more years of healthcare work or relevant experience in the mental health field.
